Moved chart titles into panel header

Jonathan Cooper requested to merge EC-154-chart-titles-in-header into master

Moved chart titles into panel header, adjusted text so that it will always fit the space available and applied the bootstrap 'well' style to the main map title on both seasonal and climate-change maps (it had been removed during a different issue)
